Invoices

UNSMIS is a worldwide medical insurance plan and, as such, requires that all invoices submitted for reimbursement meet minimum documentation standards. Properly detailed invoices help ensure timely processing and reduce the risk of delays or non-reimbursement. All claims must be submitted within two years after the date on which the bill was established.

Mandatory Information on All Invoices

All invoices must clearly and legibly include the following information:

  • Full name of the insured member
  • Date of the invoice
  • Type of treatment (for example: dermatologist consultation, physiotherapy, general practitioner visit, etc.)
  • Date(s) of treatment (for example: 2 July 2026)

N.B.: Please ensure that the healthcare provider is clearly identifiable on the invoice (i.e. the provider's name and contact details are shown). The invoice must also specify the type of medical service provided, including the relevant specialty where applicable (e.g. dermatology, gynecology, etc.), rather than a generic description such as "treatment".

Additional Requirements by Treatment Type

Invoices for session-based treatments (such as physiotherapy, psychotherapy, or similar treatments) must clearly state:

  • The total number of sessions
  • The date of each session

These invoices must be accompanied by the relevant medical prescription(s).

Hospitalization-related invoices must include a detailed breakdown of all expenses, including but not limited to:

  • Type of operation or medical procedure
  • Room expenses, clearly indicating the type of room (public ward, semi-private room, or private room)
  • Number of nights spent in the hospital
  • Doctors’, surgeons’, assistants’, and anesthetists’ fees
  • Hospital expenses (meals, operating theatre, bed charges, etc.)
  • Medical care costs (laboratory tests, examinations, imaging, and other related services)

In cases where a package price is charged, the invoice must at a minimum clearly indicate the type of room included.

Before Leaving the Medical Facility

Please ensure that all required details appear on the invoice before leaving the medical facility. If any information is missing, kindly ask the provider to:

  • Add the missing details directly to the invoice (for example, in the comments section), or
  • Attach a detailed medical report listing all services provided.

Financial Responsibility

Please note that it is the insured member’s responsibility to settle invoices directly with medical providers. UNSMIS does not pay providers on behalf of insured members.

Documents Not Accepted by UNSMIS

Please be advised that UNSMIS cannot accept the following documents as valid invoices:

  • Installment schedules for treatment
  • Statements of account issued in lieu of a standard medical bill or invoice

This practice is particularly common in North America. As these documents do not qualify as valid invoices, insured members are kindly requested to always ask the medical provider for a standard medical invoice or an Insurance Claim Form.

Failure to submit invoices that meet the above requirements may result in the return of the claim request, a delay in reimbursement, or non-reimbursement.
