Should you wish to submit a claim for expenses arising from an accident, we kindly ask you to fill out and submit a Declaration of Accident form (available here).

This is important as accidents often involve a third party, particularly road accidents. The information provided in the Declaration of Accident form allows us to verify if a third party is responsible and, if so, attempt to get reimbursement for the submitted expenses from the insurer of the concerned third party.

All this work is done in order to safeguard the long-term viability of your medical plan, by ensuring that UNSMIS does not subsidize claims for which responsibility lies with a third party.
